What insurance are you in network with?

We are out of network with Aetna and United HealthCare and will accept these carriers if your plan has out of network coverage.

We also offer private pay rates for those that are out of network.

We have NO relationship with Medicare and cannot provide Medicare-covered services, including medically necessary physical and occupational therapy, to beneficiaries.

Can I use my HSA/HRA/FSA account to pay for services?

Your HSA /HRA/FSA card can be used for all qualified services provided in accordance with your insurance plan.  Please refer to your insurance plan’s qualified medical expenses listing.

What if insurance denies my claim?

While we are happy to check your benefit information, it is ultimately your responsibility to communicate with your insurance provider regarding coverage details.  We are here to assist you with any insurance discrepancies, however you are responsible for payment in full if insurance denies your claim. If we have difficulty communicating with your insurance provider we will notify you…

We do have cash pay rates- these are not billed through insurance and therefore will not contribute to your deductible.

What if I need to cancel my appointment?

We have a 24-hour cancellation policy. Cancelling a therapy session with less than 24-hours notice is subject to a late cancellation/no show fee of $100. Insurance does not cover the cancellation fee. Cancelling a personal training session with less than 24 hours’ notice will cost one session from a prepaid package or the cost of a single session visit.

You can cancel your appointment up to 24 hours prior to your appointment time by calling 206-641-7733 or emailing info@movemend.info. From our website, you can select the schedule appointment link, create an account and delete an appointment from there.

Do I need a referral/prescription for therapy?

Washington state law does not require a referral or prescription from a physician for Physical Therapy.  Some insurance plans do require a referral for therapy to be covered, check with your insurance plan for verification.

Washington state law does not require a referral or prescription for an occupational therapy initial assessment but DOES require a prescription from a physician, naturopathic doctor or chiropractor for ongoing treatment.

What should I expect at my first occupational therapy or physical therapy visit?

As a new patient, please arrive 15 minutes early and bring your insurance information and card, a picture ID, and a referral if your insurance requires it.

If you have not already filled out intake paperwork ahead of time please expect a few minutes of paperwork collecting necessary information: legal name, contact information, diagnosis, etc.

Please bring in, or fax ahead of time, test results, physician notes and surgical reports related to the condition being treated.

Your therapist will start by asking you several questions about your symptoms. They will then perform a detailed physical assessment to identify the source of pain or dysfunction. Range of motion, strength and sensation are many of the things that may be assessed.

What should I wear for my therapy assessment?

If you have a shoulder or arm condition a tank top is ideal. For back, hip and leg conditions wear shorts or exercise tights that allow freedom of motion and therapist access to the painful body part.

What should I expect at my first personal training appointment?

You will spend some time talking with your trainer about your goals, history of exercise and recent injuries and medical conditions. Following the conversation, you will have a movement and strength assessment. You will leave with a full report of the assessment findings and a plan for meeting your health and fitness goals. We also like to know the type of music you like to listen to while exercising – we like to make your training sessions custom!

What should I wear for my personal training session?

Be ready to move and exercise! Sneakers, shorts or sweatpants and a sweat wicking shirt are ideal. We provide towels and the need to sweat!
